Cause: HYP is currently developing its fiscal resources and the turnover in this department contributed to the above anomaly arising. Recommendation: We recommend that management perform an annual physical inventory of its property and equipment and reconcile the results to the fixed assets listing and to the general ledger. Management Response: HYP is in agreement with this comment. See Corrective Action Plan attached.
Show full finding ▾Hide full finding ▴2020-002 Property and Equipment Criteria: Per the requirements, a physical inventory of equipment must be taken at least every two (2) years and reconciled with property records. Condition: We noted that HYP has not performed a physical inventory of its property and equipment for several years. Cause: HYP is currently developing its fiscal resources and the turnover in this department contributed to the above anomaly arising. Recommendation: We recommend that management perform an annual physical inventory of its property and equipment and reconcile the results to the fixed assets listing and to the general ledger. 2020-002 Property and Equipment As stated in finding, this anomaly occurred to high staffing turnover over several fiscal years. HYP has completed the hiring of appropriate resources and the finance department now consists of permanent full time staff. Staffing has been adequately trained. Due to COVID and the resulting work-from-home or remote work requirements to keep staffing safe, there have been a number of logistical and physical location changes. Physical inventory is scheduled for FYE 2022. Responsible staff member: Elaine Moya Anticipated Completion Date: September 2021
HYP did not submit its FFR within thirty days after each quarter. Cause: HYP is currently developing its fiscal resources and the turnover in this department contributed to the above anomaly arising. Recommendation: We recommend that HYP complete and submit its FFR forms within the time frame as prescribed by the funder. Management response: HYP is in agreement with this comment. See Corrective Action Plan attached.

Condition: HYP did not submit its FFR within thirty days after each quarter. Cause: HYP is currently developing its fiscal resources and the turnover in this department contributed to the above anomaly arising. Recommendation: We recommend that HYP complete and submit its FFR forms within the time frame as prescribed by the funder. Management response: HYP is in agreement with this comment. Section III Federal Award Findings and Questioned Costs 2020-003 Timely Submission of Federal Filings HYP has completed the hiring of appropriate resources and finance department now consists of permanent full time staff. Staffing has been adequately trained to execute quarterly reporting responsibilities as outlined in the accounting procedures manual and monthly close checklist. It should be noted that the online portal being was inaccessible, is a cause of late filings. Staffing will make every possible effort to file timely, barring circumstances beyond our control.
