Finding Number: 2019-001 Repeat Finding: No Program Name/CFDA Title: Child Nutrition Cluster CFDA Number: 10.553, 10.555, 10.559 Federal Agency: U.S. Department of Agriculture Federal Award Number: 7AZ300AZ3 Pass-Through Agency: Arizona Department of Education Questioned Costs: N/A Type of Finding: Noncompliance, Significant Deficiency Compliance Requirement: Eligibility CRITERIA According to the United States Department of Agriculture (USDA) Food and Nutrition Service Income Eligibility Guidelines, a child?s eligibility for free and reduced price meals under a Child Nutrition Cluster Program may be established by the submission of an annual application which furnishes such information as family income and family size. According to 7 CFR ??245.2, 245.3, and 245.6; ??9(b)(1) of the NSLA, 42 USC ?1758 (b)(1); ??3(a)(6) and 4(e) of the CNA (42 USC ??1772(a)(6) and 1773(e)), children from households with incomes at or below 130 percent of the federal poverty level are eligible to receive meals or milk free under the School Nutrition Programs. Children from households with income above 130 percent but at or below 185 percent of the federal poverty level are eligible to receive reduced price meals. Persons from households with incomes exceeding 185 percent of the poverty level pay the full price. Applications must be complete to be considered. CONDITION Income applications were incomplete. CAUSE The District did not have adequate review procedures in place to ensure applications were complete and student eligibility was properly determined. EFFECT The software system used to process income applications was not programmed to reject incomplete applications. CONTEXT For seven of 40 applications reviewed, zero income was not indicated on the applications; therefore, the applications were incomplete. The sample was not intended to be, and was not, a statistically valid sample. RECOMMENDATION The District should ensure the software program that processes the free and reduced applications has proper field restrictions to ensure complete application submissions. Finding Number: 2019-001 Program Name/CFDA Title: Child Nutrition Cluster CFDA Numbers: 10.553, 10.555, 10.559 Contact Person: Tom Wohlleber, Chief Financial Officer Anticipated Completion Date: June 30, 2020 Planned Corrective Action: Starting August 2019, the Nutrition Services Office Assistant will review online applications daily and will: ? Ensure all household members and students are in the accurate category on the online application. ? Review all children under the household member portion and identify if income needs to be specified, if no income is specified from the signer, ?No Income? will be marked on the application. ? At the end of the fiscal year, all online applications are saved onto our District network- Nutrition Services folder. In addition, completed online applications will be checked once a month by the Director of Nutrition Services to ensure applications are completed properly. Online meal application software, MySchoolApps, was a new process beginning August 2018 with only the Director of Nutrition Services having access to the software. Once a process was established and documented, the responsibility was then reassigned to the Nutrition Services Office Assistant.
