Of a sample size of forty (40) tenant files, the following information was unavailable for examination at the time of audit: Consent to release information forms were missing in nine (9) files Lead based paint forms were missing in one (1) file Copies of executed HAP contract and tenancy addendum were missing in one (1) file Our sample size is statistically valid. Known Questioned Costs: Amount is below threshold of $25,000. Cause: There is a significant deficiency in internal controls over compliance for the eligibility type of compliance related to the maintenance of tenant files. The Authority has not properly considered, designed, implemented, maintained and monitored a system of internal controls that reasonably assures the program is in compliance. Effect: The Housing Voucher Cluster is in non-compliance with the eligibility type of compliance related to the maintenance of tenant files. Eligibility - Tenant Files Non Compliance Material to the Financial Statements: No Significant Deficiency in Internal Control over Compliance for Eligibility Criteria: Tenant Files. The PHA must do the following: As a condition of admission or continued occupancy, require the tenant and other family members to provide necessary information, documentation, and releases for the PHA to verify income eligibility (24 CFR sections 5.230, 5.609, and 982.516). These files are required to be maintained and available for examination at the time of audit. Condition: Based upon inspection of the Authority’s files and on discussion with management, there were documents that were unavailable for examination at the time of audit. Authority's Response and Planned Corrective Plan: The Authority accepts the auditor’s recommendation. Management acknowledges the importance of maintaining complete and readily available tenant file documentation to support eligibility determinations and ongoing program compliance for the Housing Voucher Cluster. The documents noted as unavailable during the audit relate to file maintenance and documentation retention. Management does not believe the exceptions indicate that the sampled households were ineligible for assistance; however, the Authority recognizes that required documentation must be consistently maintained and available for audit review. In response, the Authority will implement a corrective action plan that includes increased supervisory review of tenant files, enhanced file completion checklists, periodic internal quality control reviews, and additional staff training on required eligibility documentation, including consent forms, lead-based paint documentation, HAP contracts, and tenancy addenda. The Authority will also strengthen monitoring procedures to ensure missing or incomplete documents are identified and corrected timely. Management will assign responsibility for periodic file review to Housing Choice Voucher leadership and will document follow-up actions taken. These procedures are intended to improve internal controls over tenant file maintenance and ensure continued compliance with HUD requirements, Uniform Guidance, and the applicable compliance supplement. Authority Response and Planned Corrective Action Plan: The Authority accepts the recommendation of the auditor and has initiated corrective actions to strengthen compliance with Housing Quality Standards (HQS) inspection requirements and related abatement procedures. Management identified that staffing turnover and transition-related training gaps contributed to delays in the timely processing and enforcement of failed inspection reinspections and landlord abatements during the audit period. In response, the Authority has implemented enhanced monitoring and supervisory review procedures over failed inspections and reinspection timelines to ensure compliance with HUD requirements. Management has reinforced staff training related to HQS enforcement, reinspection tracking, and Housing Assistance Payment (HAP) abatement procedures. In addition, the Authority is utilizing system generated tracking reports and management oversight tools to identify failed inspections approaching required corrective action deadlines and to ensure timely follow-up and enforcement actions are completed. The Authority believes these corrective measures will strengthen internal controls over compliance and help ensure continued adherence to HUD Housing Quality Standards requirements and related special tests and provisions compliance requirements. Views of responsible officials and planned corrective action: The Authority’s inspection process includes quality control review of inspection reports to ensure enforcement of Housing Quality Standards (HQS). This includes ensuring that all required inspections are completed as scheduled and that rental assistance is abated for any period during which a unit remains in a failed HQS status due to landlord-required repairs. If a tenant fails to make the required repairs, the Authority will initiate termination proceeds for tenant-caused damages that resulted in the unit failing the HQS inspection. The corrective action has been implemented and Wendy Herman, Vice President of Housing Choice Voucher Program, is responsible for ensuring the deficiencies have been rectified by September 30, 2025.

Context: There are approximately three thousand and sixty-six (3,066) Section 8 Housing Voucher units. Of a sample size of thirty-eight (38) files, one (1) biennial inspection was not completed in a timely manner. There were approximately two hundred and thirty-seven (237) failed inspections during the audit period. Of a sample size of twenty-four (24) failed inspections, one (1) unit did not pass reinspection within 30 days. HAP was not abated nor was the tenant transferred. Known Questioned Costs: $9,004 Our sample size is statistically valid. Cause: There is a significant deficiency in internal controls over the compliance for the special tests and provisions type of compliance related to HQS inspections. Views of responsible officials and planned corrective action: The Authority accepts the finding of eviewed its process for properly managing the Housing Quality Standards policies. This finding reflects a missed process step by the caseworker, and the Authority will put process steps in place for weekly reviews of all abated units housed in our database by the department supervisor to ensure that housing units are placed in the eligible pool of habitable housing. The corrective process steps will require the department supervisor to extract all abated units weekly and cross reference that report with the updated HQS caseworker has processed the change within 24 hours of the unit passing. Anton Shaw, Vice President of the Housing Choice Voucher Program, is responsible for implementing this corrective action by September 30, 2023 and has since enhanced internal controls immediately, as noted above, to mitigate future exceptions.
