Additionally, the state's Medicaid manual (Aged, Blind and Disabled manual, Family and Children Medicaid manual and the Integrated Policy manual) mandates that all eligibility determinations include cross-verification of applicant-provided data against reliable external sources to ensure accuracy and prevent improper payments. There were a total of 11 errors found during our testing procedures: - There was 1 error where income was incorrectly calculated or inaccurate information was entered into the case file. - There was 3 error where resources were incorrectly calculated or were not properly documented in the case file. - There were 5 errors where required information needed for eligibility determinations were not requested or not requested timely at applications or redeterminations. - There were 2 errors where a redetermination of eligibility was not performed in accordance with program requirements. Due to the nature of the populations provided from which the samples were chosen, we are unable to calculate questioned costs for the above mentioned potential eligibility and noncompliance errors. We examined 60 cases from of a total of 319,875 Medicaid claims from the Medicaid beneficiary report provided by NC Department of Health and Human Services to redetermine eligibility. These findings are being reported with the financial statement audit as it relates to Medicaid administrative cost compliance audit. These control deficiencies and noncompliance increase the risk of improper Medicaid payments, potentially resulting in overpayments to ineligible beneficiaries or underpayments to those who qualify. This is a repeat finding from the immediate previous audit, 2024-002 and 2024-003. Financial Statement Findings The County's management and other users of the financial statements do not have accurate information for decisions-making and monitoring of the county's financial position and adherence to laws, regulations, and other requirements. Errors in financial reporting could occur and not be detected. The original net present values recorded for several GASB 87 leases were incorrectly calculated. The County should keep good record of the lease schedules and make adjustments timely. The County agrees with this finding. In accordance with 42 CFR 435, documentation must be obtained as needed to determine if a recipient meets specific standards, and documentation must be maintained to support eligibility determinations. Finding 2020-001 Name of contact person: Stacy Ragsdale, Adult Medicaid Supervisor Corrective Action: "Adult Medicaid had 4 technical errors in this audit. All 4 cases were completed prior to last year's audit and subsequent training session held with staff on September 12, 2019. During this training the following elements were covered; 1) How to properly run DMV in OVS when results are inconclusive; 2) How to properly document vehicles at application and review to meet Medicaid policy rules and regulations; 3) The importance of ensuring that information must match in NCFAST with our verifications; 4) How to properly calculate and show how reserve is reduced. Previously, caseworkers were running adding machines tapes when reducing reserve. Instructions were provided on completing a DSS 5036 showing reducing reserve and documenting the correct information to show how reserve was reduced. A training session was completed October 21, 2020 for a refresher on the following topics: 1) Policy requirements to clearly document countable and non-countable resources; 2) Instruction for staff to double check online verifications for conclusive results and procedures when they are inconclusive; 3) To check determinations in NCFAST to verify income and resources are reading correctly and match NCFAST evidence. Currently the Supervisor or Lead Worker are monitoring records for these issues monthly. The second party tool was revised to include these factors.
