Questioned Costs: Due to the nature of the populations provided from which the samples were chosen, we are unable to calculate questioned costs for the above mentioned potential eligibility and noncompliance errors. There were a total of 10 errors found during our testing procedures: - There were 2 errors where income or household size was incorrectly calculated or inaccurate information was entered into the case file. In one notable instance, a failure resulted in a recipient being deemed eligible for Medicaid benefits when they were not, due to income exceeding the eligibility threshold. - There was 2 error where resources were incorrectly calculated or were not properly documented in the case file. - There were 6 errors where required information needed for eligibility determinations were not requested or not requested timely at applications or redeterminations. Federal regulations under 42 CFR § 435.956 require states to verify eligibility factors using electronic data sources where available, and to obtain documentation for unverifiable information. Additionally, the state's Medicaid manual (Aged, Blind and Disabled manual, Family and Children Medicaid manual and the Integrated Policy manual) mandates that all eligibility determinations include cross-verification of applicant-provided data against reliable external sources to ensure accuracy and prevent improper payments. Financial Statement Findings Section III. Federal Award Findings and Questioned Costs Corrective Actions for Finding 2025-001 also apply to State Award Findings. Section IV - State Award Findings and Question Costs Staff were re-trained on effective date of change, and how to verify those dates are correct in NC FAST before the continuation of case processing, along with how to verify resources and the proper way to request information and what information is vital to case processing. Policy and procedures were used to ensure staff are trained appropriately. Second party reviews will continue to occur to ensure dates are correct in NC FAST, and second party reviews have increased to target 100% of all applications. The majority of cases found in error were in error prior to this training in December of 2024. Re-training occurs monthly during staff meetings to continue to improve outcomes. More difficult eligibility determination like those involving Special Needs Trust or Pooled Trust will be assigned to senior staff for processing and will immediately be second partied by the supervisor to ensure that resources and income are accounted for properly. Any noted discrepancies will be consulted with State Operation Support Team during processing of case. Second party reviews will continue to occur to ensure accuracy on information entered, including the use of resources. Trainings were completed by December 31, 2024, monthly staff meetings have been used to reinforce those training materials. 136

In this process, the countable resources should be calculated correctly and agree back to the amounts in the NC FAST system. Any items discovered in the verification process should be considered countable or noncountable resources and explained within the documentation. There were 3 errors discovered during our procedures where resources were incorrectly calculated or were not properly documented in the case file. Of these 3 errors, one was determined to be an eligibility error with known questioned costs. The knowns and likely questioned costs due to the potential ineligible recipients mentioned above were below the threshold for reporting; therefore the questioned costs will not be included in the finding. Finding: 2022-002 Name of Contact Person: Tammy Mixon, Medicaid Supervisor Corrective Action: "Of the eight errors found during this audit, only four were repeat findings from previous audit year. Those four findings occurred prior to the training from August 24, 2021, which was as a result of the previous year findings. Therefore, the workers had not been trained on the proper procedures at the time in which these errors occurred, as they were in the previous timeframe. Further, two of the four cases mentioned were correct later in the file due to COVID 19 waivers, but the audit did not cover the timeframe in which the corrections were found. Of the four findings that were not repeat, the agency has obtained training materials from the Operation Support Team for the State of NC for training to correct. The agency rebuts that this is a repeat finding. as the findings occurred during the timeframe prior to training from previous period findings. Further, only half of the findings were the same as the previous period. The agency denies this is a Significant Deficiency, as there were eight findings out of sixty cases pulled, and half of those findings fell within a timeframe prior to training to correct the issue. These findings were discussed in the monthly Medicaid meeting September 2022. OST training materials have been obtained and will be used for training to prevent future errors. The findings occurred during the timeframe prior to training from previous period findings. This has been discussed in our monthly Medicaid meeting on August 24, 2021. As a result of that meeting and discussions of the findings, a line was added to the agency's second party review form to look for errors regarding the worker's check of all electronic sources, as well as the documentation on the file of such checks. Of the two, one of the errors was related to a case that was processed and paid in December 2021 by the State, but was for a claim with a date of service of January 2019. The eligibility decision related to this particular claim was completed in 2018 prior to our previous corrective actions. We consider the error related to this case already corrected. For the other case error training was completed FY 2020-2021, also findings were reviewed during a Medicaid meeting on September 1, 2022. Since we have hired a Quality Control Reviewer, we have been able to review more applications and recertifications to ensure accurate information is being entered. If the case is found in error this has allowed us to correct it in a timelier manner. We use a Monthly Performance Spreadsheet to track our errors to identify areas where a refresher training might be needed. Proposed Completion Date: A refresher training to be completed by December 31, 2021.

Name of Contact Person: Tammy Mixon, Medicaid/Program Integrity SupervisorCorrective Action: Medicaid Caseworkers have been trained as of 09/23/2020 with our new plan of action to ensure these cases are being handled appropriately after the COVID-19 policy is lifted. Caseworkers will continue to communicate with the Child Support Unit regarding questions that may arise when they are reading information in ACTS. The Child Support Unit will also follow up all referrals with an email to determine that a sanction has been acted upon. We will be presenting the training over again as a refresher when COVID-19 policy is lifted, as we have some vacancies and some new workers that have never been exposed to the regular rules of IV-D referrals and sanctions.Proposed Completion Date: Medicaid Caseworkers have been trained as of 09/23/2020 with our new plan of action to ensure these are being handled appropriately after the COVID-19 policy is lifted. We will be doing a refresher training after COVID-19 restrictions are lifted as well. We are working on a report to look at each case that might need a IV-D referral as well so we can get all records in line with policy and not wait until next recertification.

There were ten errors discovered during our procedures that referrals between DSS and Child Support Agencies were not properly made. Four applicants received assistance although their Child Support cases were indicating non cooperation and no good cause was established. Questioned Costs: The amounts paid out for the above in-eligible participants did not exceed the threshold for a questioned cost determination. Context: We examined 60 Medicaid applicants from the Medicaid Beneficiary report provided by NC Department of Health and Human Services to re-determine eligibility. These findings are being reported with the financial statement audit as it relates to the Medicaid administrative cost compliance audit. Effect: Four applicants received assistance for which they were not eligible. Cause: Error in reading the ACTS report, and ineffective case review process. Recommendation: Files should be reviewed internally to ensure proper information is in place and necessary procedures are taken when determining eligibility. The results found or documentation made in the case notes should clearly indicate what actions were performed and the results of those actions. US Department of Health and Human Services Passed through the NC Department of Health and Human Services Program Name: Medical Assistance Program (Medicaid; Title XIX) CFDA #: 93.778 Criteria: The Child Support Enforcement Agency (IV-D) can assist the family in obtaining financial and/or medical support or medical support payments from the child?s non-custodial parent. Cooperation requirement with Social Services and Child Support Agencies must be met or good cause for not cooperating must be established when determining Medicaid eligibility. Proposed Completion Date: Tasks are being reviewed weekly to see what tasks in NCFAST are being worked and which are not. Reports are being pulled weekly by Supervisor to see what is being worked on by each caseworker. The caseworkers have access to all reports that include their specific clients and those reports are to be worked weekly if not more often. We have put this in place at the present time to ensure that tasks and reports are being worked in a timely manner.
