Condition - One student was incorrectly approved for free meals and two students were incorrectly approved for reduced meals when they were not eligible for free or reduced meals based on their reported income on the submitted applications. Questioned Costs ? None Context - Out of the total of 50 students selected for eligibility testing, three determinations of eligibility were found to be in error. Cause and Effect - The Academy incorrectly identified one student as eligible for free meals and two students as eligible for reduced meals while their respective submitted student applications indicated they were not eligible for either free or reduced meals. Recommendation - The Academy should have proper controls in place to identify the proper eligibility status for all students. Views of Responsible Officials and Planned Corrective Actions ? The Academy will use appropriate measures to improve its application review procedures in regard to eligibility for free and reduced meals. Compliance with eligibility requirements has always been a goal of the Academy.
