2021-001 Federal Program ? Coronavirus Relief Fund, assistance listing 21.019, passed through State of Vermont Agency of Human Services from U.S. Department of Treasury. Criteria or specific requirement ? Employees eligible for hazard pay paid from the Coronavirus Relief Fund may only receive up to one week of unemployment benefits. Employers were required to verify which employees had received more than one week of unemployment benefits in order to determine how much, if any, hazard pay the employee was eligible for. Condition ? Area Resources for Individualized Services, Inc. d/b/a ARIS Solutions, Inc. required employees under its Fiscal Employer Agent designation, to complete a questionnaire in order for the employees to certify their eligibility to receive hazard pay paid from the Coronavirus Relief Fund. Employers were then also required to certify the form by signing off on the bottom. This form was the only means for Area Resources for Individualized Services, Inc. d/b/a ARIS Solutions, Inc. to know if the employee had received more or less than one week of unemployment benefits, in order to make the final determination of what the employee was eligible to receive in hazard pay, if anything. These forms were scanned in to be electronically stored as support and then the originals were securely destroyed. Certain of these forms are not available electronically and therefore there are no records of the completed and certified forms. Cause ? Area Resources for Individualized Services, Inc. d/b/a ARIS Solutions, Inc. had an error with its scanner that caused certain forms to not be scanned properly and saved electronically. Effect or potential effect ? It can no longer be verified that the employees with missing questionnaires were eligible to receive hazard pay under the Coronavirus Relief Fund. Questioned costs ? None. Context ? See Schedule of Findings and Questioned Costs for chart/table Recommendation ? We recommend Area Resources for Individualized Services, Inc. d/b/a ARIS Solutions, Inc. implement an internal control process to ensure paper files are properly scanned in and saved electronically. Views of responsible officials ? Management is aware of the need for an internal control process to verify documents were scanned successfully before the originals are destroyed and is currently working on its implementation.

Finding 2021-001 Incomplete Documentation Plan: ARIS Solutions will implement a process to verify that documents were scanned successfully before document disposal. When documents are scanned, the processor will check for document count as well as visually scan the electronic documents for accuracy. Original documents pertaining to federal grant awards will be stored onsite for 10 years before disposal.
