What is a management decision? →Required documentation of an individual?s pre-test, post-test, indicated prevention strategies, and participant exit strategy was not maintained for 1 of 12 individuals tested that completed the group sessions. Perspective information:YWCA provides counseling services to a targeted population of youth and young adults ages 11-21 that are still in high school and are experiencing early signs of substance abuse and other related problem behaviors associated with substance abuse. During fiscal year 2019 the population served included 59 individuals who completed the curriculum. YWCA is required to maintain specific documentation, listed above, to show that YWCA is meeting the requirements on providing required prevention strategies and counseling referrals to youth participants that meet certain criteria. Cause and effect:One of the files selected for an individual that completed the group sessions for the program did not include the required documentation of the individual?s pre-test, post-test, indicated prevention strategies, and participant exit strategy. YWCA failed to maintain documentation of the individual?s progress and may not have met the approved curriculum guidelines. Recommendation: Management should establish controls to ensure all required documentation is maintained for all individuals.

The documents that are required to be maintained for individuals are a consent form, participant rights, pre-test, post-test, indicated prevention strategies, and participant exit summary. This documentation allows YWCA to track the progress that is made with the sessions and ensures YWCA is following the curriculum approved by the grantor. YWCA Human Resources staff have implemented a checklist to ensure that all required forms are obtained and completed for all group sessions. A listing of all group session dates is created and a supervisor reviews to ensure forms are completed for each group session to ensure compliance.
