Special Tests and Provisions: Sliding Fee Discounts (42 USC 254(k)(3)(g); 42 CFR section 51c.303(g) and 42 CFR section 56.303(f)). Identification as a repeat finding ? None. Condition ? The Organization?s sliding fee program provides for certain discounts based on migrant status. In addition, the migrant discounts in place allowed for discounts without consideration of patient income or poverty levels as required by the Health Center Program Compliance Manual. Questioned Cost ? None Context ? A sample of 25 patients were tested out of the total population of 65,397 encounters. The sampling methodology used is not and is not intended to be statistically valid. One patient did not receive the proper sliding fee adjustment based on their income documentation. Two other patients were given adjustments based on their migrant status rather than based on income documentation. Effect ? Improper sliding fee discounts were potentially given to patients. Cause ? Elements of the Organization?s sliding fee policy is not compliant with the Health Center Compliance Manual. The Organization had taken corrective action and updated their sliding fee policy effective April 19, 2018. The errors identified were for dates of service prior to the newly implemented policy. Recommendation ? Management should verify the sliding fee policy is in compliance with the Health Center Program Compliance Manual.

BCHC has replaced its Sliding Fee Discount Scale which was based on a percentage of charges to a fixed fee schedule on 7/11/18. The fixed fee applies to all pay classes ranging from B to E and will prevent charges from being discounted below what is charged for the nominal fee in pay class A. This change will also let the patient know exactly how much the fee will be before being seen and will save the patient and out staff time by not having to collect twice (once when the patient registers and again after the patient is seen for any additional charges.) This policy also does not give preference to any patients because of their Migrant status.
