Context: The College did not use at least 7% of its Federal Work Study (FWS) funds for students employed in community service activities. Cause: Due to several staff turnovers in the Financial Aid Office, the community service waiver request was discovered to not have been completed by the submission deadline. Effect: If the College fails to meet any of the FWS community service requirements, then they will be required to return FWS federal funds in an amount equal to the difference between the amount a school should have spent for community service and the amount it actually spent. The College could also be denied future participation in the FWS Program and possibly other FWS programs. Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: CSC is in a rural area that does not afford many community service opportunities and usually files the FWS Community Service Waiver. Personnel changes caused CSC to miss the 24/25 filing deadline. CSC received the 25/26 Waiver on 06/05/2025. The 26/27 Wavier was requested 01/15/2026. CSC is creating a documented Standard Operating Procedure (SOP) on how to request the waiver and creating a Financial Aid Processing Calendar to ensure the deadline is met each year. Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: The late reporting was the result of a known FAFSA issue that began occurring with the 24/25 FAFSA Simplification and continues with the 25/26 FAFSA. The exception occurred when the student was not presented with the HS Completion Status question on the application. Students must self-certify they have a HS Diploma or Equivalent to be eligible for Federal Student Aid. CSC exported the origination to COD. COD approved the award, but CSC was unable to post the award to the student’s account because the HS Completion Status was blank. As soon as the student corrected her FAFSA, CSC posted the award and reported it to COD. The CSC FA office now receives a report with missing HS Completion Status each day and deletes federal awards until the issue is resolved preventing late COD Reporting. The Pell payment schedule considers the cost of attendance, the student's Student Aid Index and the enrollment status of the student. Cause: The College did not have proper procedures in place to capture student enrollment in summer to be packaged for Pell. Effect: Failure to properly determine and disburse Title IV funds based on eligibility for each type of aid in accordance with federal regulations may result in students receiving incorrect funds. The four students with Reporting discrepancies are correctional students that do not have access to electronic forms. This population of students must submit paper requests and have them physically routed to the Registrar’s office for processing. The Enrollment and Reporting dates were in line; the discrepancy lies in the Program Enrollment date. The Registrar is researching if the student changed programs after their Enrollment dates. For the Enrollment Reporting date discrepancy outside the 60-day requirement, we reported the correct date to NSC. The Registrar has put in a ticket with NSC to see why they reported the Enrollment Date late. A staffing change in the VP of Academic Affairs, Registrar and Financial Aid Office caused a failure in timely reporting. Also, change in staff resulted in a lack of training for the registrar who does the reporting through the National Clearinghouse (NCH), and it was noted that NCH is dropping students from the roster when students start and stop school more than twice. When the student restarts, the report is submitted to NCH; however, it is treated as a mistake and is not forwarded to NSLDS. Indication as a Repeat Finding ? Prior year finding 2019-001 Recommendation ? The College should review its procedures for reporting student enrollment status changes for students receiving Direct Loans and Pell Grants. Views of Responsible Officials and Planned Corrective Actions ? Both the registrar and director of FAO will work closely together to compare monthly reports being sent to NCH for changes. The registrar will work through the error reports received from NCH. Also, in working with IT, we have found and are correcting CIP code issues. We will run reports and compare to ensure necessary changes have been made to correct CIP codes for future terms. We will also be double checking withdrawn students who are returning and those who are not returning to ensure they are correctly reported at the first of term. The registrar plans on completing a more robust investigation of rejected NCH student records and completing follow-up, focused on the code given for the rejection in reconciliation of our student records with those of NCH. This is an alternative to past practices, as the registrar has noticed a reduction in the types and amounts of rejected records that have been received while completing the process and based on past enrollment report submissions. The current registrar never got to this stage of training as timing did not allow for a smooth transition. He has since been able to advance his knowledge and learn the proper handling of rejected records in a more timely manner.

The findings from the schedule of findings and questioned costs are discussed below. The findings are numbered consistently with the numbers assigned in the schedule. Student Financial Assistance Cluster Corrective Action Plan for Enrollment Reporting Finding 2020-001 The Financial Aid and Registrar?s Offices are creating a process to conduct a secondary check of report submissions to correct the issues found during audit. The process involves running the enrollment report that is sent to clearing house and comparing the report to the previous month for any changes from census date to first month of the term on the initial report, then month to month thereafter. The process was established in March 2021 and will compare enrollment status changes monthly. These changes will then be checked directly in NSLDS. If an update to NSLDS needs to be made it will be done at that time to meet the reporting requirements. This process should not only locate status changes, per student, but graduation dates, enrollment begin and end dates, and CIP codes to report correctly to NSLDS.
