One instance of verbal income verification was noted in the System?s sampling of free and reduced-price applications. Criteria: Income verification is required on a sample of free and reduced-price applications and evidence of that verification must be written. Cause: The Manager confused requirements for acceptable evidence for income verification with what is considered an acceptable response in determining response rate for sampling purposes. Effect: Since proper verification procedures were not followed, there is a possibility that ineligible students may have received benefits through this program. Recommendation: Management should ensure that written evidence is obtained during the application verification process. Views of Responsible Officials and Planned Corrective Actions: Fayetteville City Schools confirmed the requirements with the state pass-through agency and concurs with the finding. Steps were taken immediately to correct procedures for future verifications.

MANAGEMENT?S CORRECTIVE ACTION PLAN Year Ended June 30, 2020 Fayetteville City Schools respectfully submits the following corrective action plan for the year ended June 30, 2020. FINDINGS ? FINANCIAL STATEMENTS AUDIT None FINDINGS AND QUESTIONED COSTS ? MAJOR FEDERAL AWARDS PROGRAM AUDIT SIGNIFICANT DEFICIENCY DEPARTMENT OF AGRICULTURE FINDING NUMBER 2020-001: Condition and Criteria: Income verification is required on a sample of free and reduced-price applications and evidence of that verification must be written. One instance of verbal income verification was noted in the System?s sampling. Recommendation: Management should ensure that written evidence is obtained during the application verification process. Planned Corrective Action: Written evidence will be obtained for all application verifications. Responsible Person: Director of Schools, Bill Hopkins Anticipated Completion Date: Corrective action will be implemented immediately for all future income verifications. If the Department of Education or Agriculture has questions regarding this plan, please call Bill Hopkins at (931)433-5542.
