Criteria or specific requirement: The USDA note requires the Organization provide the following reports annually in addition to the Financial Statement Audit: current insurance policy, current list of Officers, current year approved budget, and debt reserve statement. The Organization is also required to send quarterly balance sheet and statement of budget reports due to re-amortization in 2019. Condition: The Organization must have a policy implemented to ensure it meets all annual and quarterly reporting requirements. Effect: Failure to comply with requirement provided in the Compliance Supplement. Cause: The Organization does not have a policy implemented to ensure it meets requirements regarding quarterly and annual reporting. Recommendation: Implement a policy that ensures the Organization meets all quarterly and annual reporting requirements.

Condition: The Organization must have a policy implemented to ensure it maintains the minimum balance prescribed above in the debt reserve fund. The Organization must also make payments from a general operating account as long as it has the applicable funds to do so. Effect: Failure to comply with requirement provided in the Compliance Supplement. Cause: The Organization does not have a policy implemented to ensure it meets requirements regarding minimum balance and note payment source. Recommendation: Implement a policy that ensures the Organization is maintaining minimum debt reserve and servicing payments from its general operating account provided it has sufficient funds.
