Questioned Costs: Context: Effect: Identification of a repeat finding: Cause: Due to the nature of the populations provided from which the samples were chosen, we are unable to calculate questioned costs for the above mentioned potential eligibility and noncompliance errors. The deficiencies and noncompliance reported above are caused from ineffective record keeping and case review processes due to insufficient training of staff, lack of oversight from management, and/or inadequate communication between departments resulting in inconsistencies in data sharing and delayed information exchanges. These control deficiencies and noncompliance increase the risk of improper Medicaid payments, potentially resulting in overpayments to ineligible beneficiaries or underpayments to those who qualify. Federal regulations under 42 CFR § 435.956 require states to verify eligibility factors using electronic data sources where available, and to obtain documentation for unverifiable information. Additionally, the state's Medicaid manual (Aged, Blind and Disabled manual, Family and Children Medicaid manual and the Integrated Policy manual) mandates that all eligibility determinations include cross-verification of applicant-provided data against reliable external sources to ensure accuracy and prevent improper payments. We examined 60 cases from of a total of 641,740 Medicaid claims from the Medicaid beneficiary report provided by NC Department of Health and Human Services to re-determine eligibility. These findings are being reported with the financial statement audit as it relates to Medicaid administrative cost compliance audit. There were a total of 14 errors found during our testing procedures: - There were 3 errors where income or household size was incorrectly calculated or inaccurate information was entered into the case file. - There was 1 error where resources were incorrectly calculated or were not properly documented in the case file. - There were 6 errors where required information needed for eligibility determinations were not requested or not requested timely at applications or redeterminations. - There were 1 errors where applicants/beneficiaries not reviewed timely and determined to be eligible for Medicaid when their SSI benefits were terminated. - There were 3 errors where a redetermination of eligibility was not performed in accordance with program requirements. Documentation standards and quality review processes are already in effect, with ongoing monitoring. Angel Carpenter and Goldie Davis - Medicaid Supervisors All Medicaid caseworkers will complete targeted refresher training on key eligibility and budgeting rules, including the use of online verification systems, accurate income and deduction calculations, household composition, recertification processes, and proper case documentation standards. Training will be delivered through a combination of state Learning Gateway courses, webinars, and internal sessions, with knowledge checks to confirm understanding. Staff will be reminded that “if it’s not documented, it didn’t happen.” Standardized documentation templates have been created and are now required for all cases to ensure thorough, clear, and consistent case notes. Second-party case reviews will continue and be expanded as needed to monitor ongoing accuracy. Case errors and lessons learned will be regular agenda items at monthly staff meetings, with emphasis on double-checking determinations before authorizing or releasing cases in NC FAST. Dedicated weekly time will be protected for staff to work pending verifications and system reports, with supervisory review. Section II - Financial Statement Findings 8/14/2025 Nikki Stanton, Finance Director The Nash County Finance Director was appointed effective April 14, 2025. Since that time, Finance has undertaken the following measures to strengthen operations and internal controls: • Reclassified job duties to better align responsibilities with organizational needs and improve efficiency. • Implemented additional internal controls to enhance the reliability and accuracy of financial processes. • Recruited and onboarded a dedicated Accountant to support the Accounting Manager. These changes have enabled the Accounting Manager to concentrate on performing timely reconciliations and preparing accurate journal entries, thereby improving the overall timeliness and quality of financial reporting. In accordance with 2 CFR 200, management should have an adequate system of internal controls procedures in place to ensure an applicant is properly determined or redetermined for benefits. The Board spent funds that were not available for those respective functions and funds. In this process, the countable resources should be calculated correctly and agree back to the amounts in the NC FAST system. Any items discovered in the verification process should be considered countable or non-countable resources and explained within the documentation. The Finance Director will work with the Accountant to calculate and update the EMS net receivables each year to ensure proper posting to the General Ledger, working with information from the County’s billing and collection agency. Review of policy for exparte process and system reviews. US Department of Health and Human Services Passed through the NC Department of Health and Human Services Program Name: Medicaid Assistance Program (Medicaid; Title XIX) AL# 93.778 Finding 2022-002 Inaccurate Information Entry SIGNIFICANT DEFICENCY Eligibility Criteria: In accordance with 42 CFR 435, documentation must be obtained as needed to determine if a recipient meets specific standards, and documentation must be maintained to support eligibility determinations. In accordance with 2 CFR 200, management should have an adequate system of internal controls procedures in place to ensure an applicant is properly determined or redetermined for benefits. Condition: There were 18 errors discovered during our procedures that inaccurate information was entered when determining eligibility. Questioned Cost: There was no affect to eligibility and there were no questioned costs. Context: We examined 60 of a total of 1,105,616 Medicaid claims from the Medicaid beneficiary report provided by NC Department of Health and Human Services to re-determine eligibility. These findings are being reported with the financial statement audit as it relates to Medicaid administrative cost compliance audit. Effect: For those certifications/re-certifications there was a chance that information was not properly documented and reconciled to NC FAST and a participant could have been approved for benefits for which they were not eligible. Identification of a repeat finding: This is a repeat finding from the immediate previous audit, 2021-002. Cause: Ineffective record keeping and ineffective case review process, incomplete documentation, and incorrect application of rules for purposes of determining eligibility. IMC Supervisor III, Lyn Saunders- IMC Supervisor II Corrective Action: "Training to be provided to all caseworkers to include TWN and OVS learning gateway webinars. Review of policy for exparte process and system reviews. Training to also include Income Policy, how to review for self-employment income and utilize the income wizard to enter weekly, bi-weekly and monthly income amounts so the system will calculate the income and leave less room for user error. Documentation of what income is being evaluated to also include why certain incomes are not counted. Training to include review of Household Composition, tax filing status and how to review the determinations of each case before completing/ releasing auto holds. Target 2nd parties will be complete at 2 per worker per week of cases processed within the month. Feedback shared with worker to ensure training was effective. Training to be provided to all caseworkers to include TWN and OVS learning gateway webinars. Training will also include Income Policy and updated Job Aids will be provided during Nov. 2022 monthly meeting for evidence entry on the dashboard. The Child Support Enforcement Agency (IV-D) can assist the family in obtaining financial and/or medical support or medical support payments from the child?s non-custodial parent. Cooperation requirement with Social Services and Child Support Agencies must be met or good cause for not cooperating must be established when determining Medicaid eligibility. A laminated desk reference will be provided at the time of training, this will have examples of when a referral is needed along with how to enter the referral within NCFAST. Update documentation template to ensure IV-D referral reason is documented within case notes as to why the referral was needed or not. This will be shared with staff at the training provided and guidelines presented as to how this is a required documentation addition. Medicaid Supervisor, Team Leads and Staff Development will complete target 2nd parties on 2 cases per worker per week that have been processed within the same month to ensure each worker is following the process of reviewing ACTs and submitting IV-D referrals when required. This information was not shared with other departments in DSS from which the recipient was also receiving benefits. Recommendation: Any State communications related to applicants? benefits received by any DSS department should be shared with all areas from which the participant receives benefits. Adult Medicaid Supervisor Corrective Action: "Medicaid caseworkers will receive additional and/or refresher training to include but not limited to the entry of evidence in the NCFAST System. This will include the review of electronic verifications such as AVS, OVS and TWN and the entry of evidence on the dashboard. This will also include outside verification for life insurance and of tax value that is unable to be determine by the electronic verification systems. Accuracy check point will be completed by the caseworker by reviewing the case determinations to ensure correct income and resources values are entered correctly and counted correctly for each household member on the dashboard. They will ensure that the cash value or tax value amount is updated at each review and correctly entered at the time of application. Even if the resource is determined to be a noncountable resource, the value will be updated accordingly. Second Party reviews will continue to be conducted to monitor continued progress and to ensure policies and procedures are correctly followed by caseworkers. Documentation templates have also been created and put into place to assist in ensuring cases are thoroughly documented. Workers are given updated Job Aids as well as guides/desk references created from webinar trainings and/or policy. Staff will be reminded to make certain the verifications entered reflect correctly within NCFAST under the Household Resource Tab of the eligibility check and comments are entered for all evidence showing the date, source and base period verified. There will be additional and/or refresher training in the area of conversion of earned income and the proper sources of income verification. There is an automated budget sheet to assist in this area that is used by staff as a comparison to the calculations completed within NCFAST. Second Party review will continue to be conducted by the Supervisor, Lead Worker, and Staff Development monthly to monitor the items noted within the Corrective Action Plan. Discussion will occur with staff if errors are discovered and covered as a unit within scheduled unit meetings. Discussion will include noted errors, policy, reminders of reviewing determination decisions to ensure they are correct prior to authorizing or releasing determinations from hold, and examples of correct actions. US Department of Health and Human Services Passed through the NC Department of Health and Human Services Program Name: Medicaid Assistance Program (Medicaid; Title XIX) CFDA# 93.778 Criteria: In accordance with 42 CFR 435, documentation must be obtained as needed to determine if a recipient meets specific income standards, and documentation must be maintained to support eligibility determinations. In accordance with 2 CFR 200, management should have an adequate system of internal controls procedures in place to ensure an applicant is properly determined or redetermined for benefits. Condition: There were six errors discovered during our procedures that inaccurate information was entered when determining eligibility. Four of the six errors were inaccurate income calculation; One of the six errors was incorrect household size calculation. One of the six errors resulted in a applicant receiving benefits from the wrong class of the program. Questioned Cost: The amounts paid out for the above ineligible participants did not exceed the threshold for a questioned cost determination. Context: We examined 60 Medicaid applicants from the Medicaid Beneficiary report provided by NC Department of Health and Human Services to re-determine eligibility. These findings are being reported with the financial statement audit as it relates to Medicaid administrative cost compliance audit. Effect: For those certifications/re-certifications there was a chance that information was not properly documented and reconciled to NC FAST which could affect income. Therefore a participant could have been approved to receive assistance for which they were not eligible. Corrective Action Plan

Name of contact person: Angel Carpenter, Family & Children?s Medicaid Supervisor Corrective Action: Medicaid caseworkers will receive additional and/or refresher training to include but not limited to running online data (OVS) when required, reviewing case determinations to ensure correct income and household size are being counted for each household member actively receiving on case(s), and accuracy of data entered onto dashboard. Second Party reviews will continue to be conducted to monitor continued progress and to ensure policies and procedures are correctly followed by caseworkers. Documentation templates have also been created and put into place to assist in ensuring cases are thoroughly documented. Case errors will be included on the Agenda for upcoming Staff Meeting and discussion will include review of accuracy/double checking determination decisions to ensure they are correct prior to authorizing or releasing determinations from hold on cases in NC FAST and ensuring correct income and household compositions are correct on determinations prior to authorizing or releasing cases from hold. Proposed completion date: Management will continue to monitor progress of these issues, as well as any others found, and address with training, staff meetings, etc. as needed when discovered.

Corrective Action Plan

Name of contact person: Audrey C. Jones, IMS II Corrective Action: Medicaid Caseworkers will receive additional and/or refresher training in the areas of entering the correct verifications and end dating evidence in NCFAST for resource verifications to match verifications received. Staff will be reminded to make certain the verifications entered reflect correctly within NCFAST under the Household Resource Tab of the eligibility check and comments are entered for all evidence showing the date, source and base period verified. There will be additional and/or refresher training in the area of conversion of earned income and the proper sources of income verification. There is an automated budget sheet to assist in this area that is used by staff as a comparison to the calculations completed within NCFAST. Second Party review will continue to be conducted by the Supervisor and Lead Worker to monitor the items noted within the Corrective Action Plan. Discussion will occur with staff if errors are discovered and covered as a unit within scheduled unit meetings. Discussion will include noted errors, policy, reminders of reviewing determination decisions to ensure they are correct prior to authorizing or releasing determinations from hold, and examples of correct actions. Proposed completion date: The noted areas are currently being reviewed by Second Party review. Management will continue to monitor the progress of this issue and modify as needed. Medicaid caseworkers will receive additional training in the above area. The Supervisor and Lead Worker will review second party review forms monthly to determine if correct actions are being taken in the area of resources and earned income calculations.
