There were Fourteen (14) technical errors discovered during our procedures that resources in the county documentation and those same resources contained in NC FAST were not the same amounts or files containing resources were not properly documented to be considered countable or non-countable. The errors were as follows: Six (6) cases lacked substantiating documentation and/or inaccurate resource calculations, Five (5) cases lacked substantiating documentation and/or inaccurate income budgeting/calculation, One (1) case was improperly forced, and Two (2) cases contained errors in the input of household size evidence. Questioned Costs: There was no affect to eligibility and there were no questioned costs. Context: We examined 91 Medicaid transactions out of 110,178 to re-determine eligibility Medicaid claims from the Medicaid beneficiary report provided by the NC Department of Health and Human Services. These findings were disclosed in a separately issued spreadsheet to the North Carolina Department of Health and Human Services and are being reported with the financial statement audit as it relates to Medicaid administrative cost compliance audit. Effect: For those certifications/re-certifications there was a chance that information was not properly documented and reconciled to NC FAST which affect countable resource and a participant could have been approved for benefits that they were not eligible. Corrective Action: Trainings, Policy, and Procedures are being developed for all Medicaid caseworkers to follow. Medicaid workers will need to make sure they are working the SSI Termination report to eliminate any future paybacks. Also, a training on SSI Termination / Ex Parte will be conducted with all Medicaid workers, including NC Fast Learning Gateway Training “Supplemental Security Income (SSI) Course”. Proposed Completion Date: June 30, 2026. Certain controls are currently being created and reviewed. Management will continue to monitor the progress of this issues and modify the controls as needed.

Technical errors consist of the following: Cases lacked substantiating documentation and /or inaccurate resource calculations, cases were improperly forced, and case lacked proper verification of facts. Documentation Training Power Point will need to be utilized for all Medicaid workers when documenting each case. Application/Recertification check list will be used for applications and recertifications. Supervisor and Lead Worker will continue to review all pending Medicaid applications, and complete 2nd Party reviews once applications are completed. NC Fast Learning Gateway trainings and New employee trainings will be conducted. Medicaid workers will need to make sure they are working the SSI Termination report to eliminate any future paybacks. Workers will continue to have Round Table Meetings to discuss policy, administrative letters, and cases. Medicaid workers will keep the case record accurate, verifications, and reserve calculations correct. The county finance office will also be participating in the review process. Corrective Action: There were (2) errors discovered during the procedures that were inaccurate budget calculations in NC Fast. The values entered in NC Fast evidence and used in the eligibility determination did not match the supporting documents or was lacking any substantiating documents. No eligibility errors were found. New Trainings, procedures, and controls are being developed for all Medicaid caseworkers to follow. Applications and Recertifications will continue to be reviewed by Supervisor and Lead Worker. NC Fast Learning Gateway Refresher trainings and new employee trainings: MAGI: Income Determination and Medicaid Ages, Blind, and Disabled (ABD) Income Computation. Manual Budgets and NC Fast Budgets must be used and reviewed to ensure both budgets match. All income must be verified if terminated income and not showing in NC Fast. Income also must be documented in NC Fast of the verification used for calculation of income. Corrective Action Plans will be continued for all workers who are found with trending errors on quarterly 2nd Party Review Spreadsheet. Condition: There were six (6) errors discovered during our procedures that resources in the county documentation and those same resources contained in NC FAST were not the same amounts or files containing resources were not properly documented to be considered countable or non-countable. The errors were as follows: Two (2) cases did not have accurate resource calculations, Two (2) cases did not have accurate budget calculations, One (1) case lacked required compliance component, and One (1) case inaccurate deductible calculation. Questioned Costs: There was no affect to eligibility and there were no questioned costs. Context: Of the 162,488 case files, we examined 60 Medicaid applicants to re-determine eligibility. These findings were disclosed, in detail, in a separately issued spreadsheet to the North Carolina Department of Health and Human Services and are being reported with the financial statement audit as it relates to Medicaid administrative cost compliance audit. Identification of a repeat finding: This is a repeat finding from the immediate previous audit, 2021-001. Effect: For those certifications/re-certifications there was a chance that information was not properly documented and reconciled to NC FAST which affected countable resource and a participant could have been approved for benefits that they were not eligible. Cause: Ineffective record keeping and ineffective case review process, incomplete documentation, and incorrect application of rules for purposes of determining eligibility. Recommendation: Files should be reviewed internally to ensure proper documentation is in place for eligibility. Medicaid caseworkers will receive new tools ?Check off list? to assist with Applications and Recertifications. Additional training will be provided and the newly created ?Documentation Template? for Applications, Recertifications, and Change of Circumstance will be used by all Medicaid workers. Supervisor will complete all Second Party Reviews for each quarter for all Medicaid workers to determine if correct tools that were provided are being used. Workers will work as a group each day to review all applications that has been completed before scanning into system. Workers and new workers will complete trainings as a group and individually. Workers will continue to have Round Table Meetings to discuss policy, administrative letters, and cases. All workers will be retrained on what information should be maintain in case files, and the importance of keeping the case record accurate and reserve calculations correct. The County finance office will also be participating in the review process. Proposed Completion Date: November 1, 2022. Certain controls are currently being created and reviewed. Management will continue to monitor the progress of this issue and modify the controls as needed.
