Repeat Finding Yes Action Taken The staff accountant does the payroll. We save a backup timecard report each payroll that we are paying from the approved by the employee's supervisor. We also have an internal worksheet that we use to document any changes that are made. Once the accountant is done with her review the controller will do the second review before we finish processing the payroll enforcing internal controls that are in place and being followed.

Repeat Finding Yes Action Taken As of May 1, 2023, the Director of Revenue and/or designee runs a slide fee report daily the reflects everyone that applied the day before. Billing personnel separate the report and audit the files to ensure the correct slide has been applied. The paper application is forwarded to the billing department for a third audit if the application is in order and has been uploaded into the patients' files and then it is manually filed. The CFO will perform random audits and will present to the Board and CEO a quarterly report with the results. In addition, As of July 1, 2023, the Access Coordinators who normally collect and process the slide fee discount program applications are now reporting to the finance department with the Director of Revenue supervising and training staff. In addition, there will now be a Lead Access Coordinator in very clinic that will audit and perform additional training where necessary.

This position also reviews current slide applications for patients that are sacheduled for upcoming appointments to ensure paperwork is current or if paperwork is outdated a new application is received. Sliding Fee Scale Discount Recommendation We recommend that management review the control system surrounding payroll changes to ensure that payroll changes are properly reflected in the payroll records. Action Taken 1. It is noted that the error which resulted in this finding occurred in February 2020 which was before the Organization implemented additional controls to their payroll process. The Organization has since implemented the additional controls during the review of the pre-processed payroll registers that includes the comparison of established pay rates to calculated pay rates based on hours and gross pay. Any pay rate changes must be supported by approved documentation. 2020.002 ? Sliding Fee Scale Discount Recommendation The Center should ensure that internal controls are in place to effectively ensure that patients receive the correct sliding fee discount. Action Taken 1. The Organization has established processes to review sliding fee discount applications daily. The Revenue Cycle Manager runs reports and assigns each billing staff with a list of applications for review. Any findings are communicated with the individual staff member who entered the sliding fee information. The information is also communicated to the Clinic Managers for additional training and corrective action opportunities. This resulted in the exempt employee being overpaid for paid time off hours in addition to their normal salaried amount for one payroll period. Questioned Costs None. Action Taken The organization has recently revised their Sliding Fee Discount Program Policy and Schedules. The Sliding Fee Discount Program Policy has been assigned to every staff member in their training platform, Relias. The organization is also working to develop monthly audit procedures to identify inconsistencies in the application of their sliding fee program and will use findings from these audits to identify areas that may require additional training. Anticipated implementation date of 06/01/2020.
