Finding #2025-001 – Lack of Segregation of Duties Over Payroll (Prior Year Finding #2024-001) Condition: Management is responsible for the design, installation and maintenance of an appropriate system of internal control. Proper segregation of duties is an important aspect of any control system. The limited size of the District’s office staff prevents the ideal segregation of functions over payroll. The Payroll and Human Resources Administrative Assistant is the only employee responsible for entering employee salaries and hourly pay rates and has access to process payroll. Effect: Errors or intentional fraud could occur and not be detected timely by other employees in the normal course of their responsibilities as a result of the lack of segregation of duties over payroll. Cause: Limited number of personnel. Criteria: Segregation of duties is an aspect of internal control intended to prevent or decrease opportunities of intentional and unintentional errors and fraud. Duties and responsibilities are properly segregated if no single individual either has control over all phases of a transaction or has the ability to both make and conceal an error, whether such error is intentional or unintentional. Recommendation: We recommend employee salary and hourly pay rates be reviewed and approved by someone independent of the payroll process. We also recommend an independent review and approval of payroll registers prior to distribution or direct deposit processing. Response: We agree with this finding. Due to staffing limitations, full segregation of payroll duties is not currently feasible, however, the District will implement additional compensating controls, including independent review and approval of employee salaries and hourly rates and payroll registers prior to processing.

Finding #2021-001 ? Segregation of Duties (Prior Year Finding #2020-001) Condition: Management is responsible for the design, installation and maintenance of an appropriate system of internal control. Proper segregation of duties is an important aspect of any control system. The limited size of the District?s office staff prevents the ideal segregation of functions. The Business Manager is the only employee that records transactions in the general ledger, records cash receipt adjustments in the general ledger, prints accounts payable checks using electronic signatures, performs bank reconciliations, and has access to process payroll. Effect: Errors or intentional fraud could occur and not be detected timely by other employees in the normal course of their responsibilities as a result of the lack of segregation of duties. Cause: Limited number of personnel. Criteria: Segregation of duties is an aspect of internal control intended to prevent or decrease opportunities of intentional and unintentional errors and fraud. Duties and responsibilities are properly segregated if no single individual either has control over all phases of a transaction or has the ability to both make and conceal an error, whether such error is intentional or unintentional. Recommendation: We recommend that the Board of Education and the District Administrator continue to monitor the transactions and the financial records of the District. We also encourage the District to continue to identify cost effective opportunities to improve the design of the internal control structure. Response: We agree with this finding but do not believe it is cost effective to increase the office staff in an attempt to bring about a more effective segregation of duties. The District Administrator approves purchase orders and the Board of Education approves monthly accounts payable checks. Also, the Building Principals review payroll timesheets prior to processing payroll. The Board of Education, District Administrator, and Building Principals will continue to monitor transactions of the District.

Child Nutrition Reporting - #10.559 (Prior Year Finding #2020-004) Condition: The District incorrectly reported the number of meals served for December 2020, January, 2021 and March, 2021. Meals reported should agree to the number of meals served. Effect: 324 breakfasts and 324 lunches were reimbursed at the 2020 reimbursement rates instead of the 2021 rates. Cause: Meals for December 28, 2020 through January 1, 2021 were prepared and delivered to students in December. There were 324 breakfasts and 324 lunches delivered in December for consumption on January 1, 2021 that were reported on the December, 2020 claim. Also, when the March, 2021 claim was originally prepared, the District inadvertently under-reported ADA meals by 1,564, breakfasts by 4,789 and lunches by 7,775. This error was discovered by the District when preparing for the annual audit. A corrected claim for March was filed on September 2, 2021 and reimbursed on September 13, 2021. Criteria: The number of meals reported should agree to the number of meals served. Recommendation: The District should develop procedures to ensure the correct number of meals are being reported. Response: Beginning in April, 2021, the District began having an additional Food Service Department employee review food service claims to help ensure the correct number of meals are being reported.

Meals reported should agree to the number of meals served. Effect: The District received aid for more meals than were served. Cause: The District prepared two days? worth of meals for students at one time. The District would tick off each meal prepared on the first day and would report the same number of meals for the second day. Meals were prepared on April 3 and correctly recorded. The District inadvertently recorded the same number of meals for April 4, however, this was a Saturday and no meals were prepared. The District also inadvertently missed meals served on April 1 at one of the locations as this date was included on the same page as the March 30 and 31 meals and was missed when adding together April meals. The net effect is that 146 meals were reported as being served that were not. Criteria: The number of meals reported should agree to the number of meals served. Recommendation: The District should develop procedures to ensure the correct number of meals are being reported. Response: April was the first full month of preparing meals while in virtual learning. Processes and procedures were improved as the District became more comfortable with this process. The District now has procedures in place to ensure the correct number of meals are being reported.
